Join experts in the field of diabetes prevention and treatment to learn about and discuss innovative practices to improve care and provide culturally sensitive and community-specific services for vulnerable people with diabetes.
Attendees of the Community Health Summit will have the opportunity to connect with and learn from leaders in the field of diabetes prevention and treatment as they share their innovative approaches to achieving positive health outcomes for patients, as well as their challenges, and course corrections.
Findings from the workshop, detailing programs successes, limitations, and potential opportunities, will be captured, published, and disseminated to a broader public health audience.

REGISTRATION
We are able to accommodate two representatives per health center/organization.
Attendees should have insight into the successes and challenges of the program.

TRANSPORTATION AND HOTEL
Direct Relief will cover hotel costs for two nights and reimburse travel up to $800.
Registrants also have the option to have their their flight booked directly by Direct Relief.

Transportation
The Santa Barbara Airport (SBA) is located just 1 mile from the Direct Relief office and workshop hotels.
Los Angeles International Airport (LAX) is 90 miles south and will require a car rental or round-trip ticket on the Santa Barbara Airbus. The Airbus drops off is in downtown Goleta (city adjacent to the Direct Relief office) and is a short Lyft or Uber ride to the workshop hotels.
The Goleta Amtrak Station (GTA) is located in between the Direct Relief office and hotels.
Hotel
A block of rooms reserved at two nearby hotels. Room reservations must be received by 12:00 PM PST, June 25th.
